The MI Choice Waiver Program is a community based, In-home service program designed to help individuals stay in their go e or transition to an eligible community setting.
These settings includes: A rented, owned or family own home, an unlicensed assisted living home, licensed (AFC) Adult Foster Care Home or a licensed home For The Aged.
MI Choice Waiver Program includes prescreening, assessments, support coordination and direct purchase or services.
This program does not provide 24 hour care

Who Is Eligible
Who Is Eligible
- Are medically appropriate for a nursing home
- Are Medicaid eligible
- Are 18 years of age or older with a disability who would otherwise require a nursing home
- Reside in the county areas
How To Participate
How To Participate
All individual referred to MI Choice receive a telephone intake interview to assist in determining individual eligibility for in-home assessment.
If an individual is eligible, a registered nurse and social worker visit the person home for an in depth assessment.
The assessment evaluates the individual’s’ physical & mental health functional status, informal and formal support, home environment, financial resources and third party support, such as insurance.
If an individual is not eligible, he or she is assisted through other referrals.
